I had a WAAF Officer have an ND with a Browning beside me on a range... "Make safe" order given, but mag went in before slide went forward...!!
We were advancing from 25 to 20m, she had her finger on the trigger, and BANG... gravel kicked up 3 ft in front of me..so she wasn't even keeping it pointing downrange..!!! :o

25 years ago I was training some work colleagues, when a lady had a misfire in my .32 Webley, so she turned round and said "It's not working...." That was the first and last time that I hit a lady.

But obviously rifles are much safer...
