Yes, just put the slots down as you've listed them.

Annoyingly the description part of the firearm varies from police force to police force, some will just issue it with 'rifle' others ask for a specific action type. This is further complicated by there being several different computer systems throughout the country which form the National Firearms Licensing Management System (NFLMS), and some allow 'new' action types to be entered, others just allow the FEO to select one from a drop down list, so you'll get some FAC's with 'straight-pull' as the action type and some as 'bolt-action' even though they cover the same make/model gun wallhead

In fairness there are only action options available for the 7.62x39mm rifle i.e. straight-pull/bolt-action and trigger-release (lots of fun but quite expensive!), as far as i'm aware there are only straight-pull 7.62x54R rifles but i'd happily be corrected on that.
Mosin Nagant series of bolt action rifles ?

There are also under-lever 7.62x54mmR rifles, Snayperskaya just started a recent thread discussing the Winchester 1895. However, these rifles are extremely rare and not common like Mosina's.

Firearm Type is 'rifle' or 'muzzle loading pistol' etc, don't mention the action type unless you want to be restricted. This gets mentioned later once you have purchased a specific action type.

That's the best thing to ask for on your application form but some FLD's insist on an action type being specified when you're making the request, sadly including mine. It made getting a VZ58 MARS on my FAC a 3 month saga as their version of the NFLMS doesn't have MARS/trigger-release as an option for them to select and they can't just input their own action type into the system (supposedly) 8-)
