From reading between the lines of various reports, it was the 2nd shot which was the ND so she must have had poor muzzle control and poor trigger discipline to shoot someone waist height a few feet behind/to the side of her. Sounds like she failed to break the gun and unload before turning away from the safe direction of fire.

A teenager is fighting for her life after being shot at a clay pigeon shoot, it is reported.
The unnamed 19-year-old is believed to have been hit at close range in Tarbert, Argyll, in Scotland in a “terrible accident” by a woman who is also a member of the gun club.
Emergency services rushed to the scene last Sunday and the teenager was given treatment before being taken to hospital with her current condition unknown. It is believed that she had been hit by a stray shot from a woman following a planned round being fired off.
Police are investigating the incident at a shooting club in Scotland where an event was taking place and started around 11am. Detectives are not treating it as “suspicious”. The woman who fired the shot is thought to be in her 40s and was taking part in the competition.
Four people, including one-year-old boy, killed in horrifying shooting
A source told The Sun: “It was a complete freak accident - everyone is absolutely devastated. There’s nothing untoward about it at all. It was a freak thing that happened. The gun just went off. There’s more than one cartridge at once, obviously a double barrel shotgun.”
Another insider added: “After the first shot was fired the gun was at waist height and went off a second time - they were just feet away from each other when it happened. It hit the younger woman in the hip and stomach. There was lots of blood. Everyone was totally shocked by what happened." The person added that it appeared as though the gun had gone off accidentally in a "horrible accident".
Local Councillor Alastair Redman has reportedly said he was shocked when he heard the news and his thoughts were with the teen and her family. He also said that incidents like this at a club with experienced instructors was very unusual.
A spokesperson for the Scottish Ambulance Service confirmed that an air ambulance was sent to the scene and a patient was taken to the Queen Elizabeth University Hospital. A police spokesperson reportedly said: “Around 12.35pm on Sunday, 3 December, 2023, police received a report of a 19-year-old woman being injured by a firearm at a gun club in the Tarbert area. The woman was taken to the Queen Elizabeth University Hospital for treatment and enquiries into the incident are ongoing. There are not believed to be any suspicious circumstances.”

My thoughts on it ... maybe a misfire and the shooter turned before breaking at which point it finally went off.

Years ago I was at a straw-baler clay shoot and 2 people who had never shot a gun before were attending as guests. They didn't shoot well all day when one of them finally hit a clay it happened to be on one of the stands that didn't have a cage. She hit it with the first barrel and was so excited she turned around and muzzle swept 50% of the rest of the squad with the second barrel before the person helping her had a chance to react and get the gun turned away.
Ironically that was also the first stand where the person who was helping her had loaded both barrels, previous to that only one cartridge had been loaded at a time for this exact scenario.
Fortunately she didn't have her finger on the trigger. Things like this can happen in a fraction of a second and it can be difficult in balancing a fun and enjoyable environment while maintaining strict safety procedures.
I hope the victim recovers fully.

If it was a competition, you would have thought the shooter would be experienced in which case she should have counted to 30 and then broke the gun open. You NEVER turn around and face people with an unbroken loaded gun.
